We offer culture-expanded autologous MSC therapy and PRP for the management of chronic musculoskeletal conditions in dogs, delivered peripatetically or at a practice with access to outpatient CT. Our protocols are grounded in published clinical evidence and applied with a targeted, holistic approach.
Conditions such as hip and elbow dysplasia and osteoarthritis are progressive and incurable. Conventional management — whether surgical or pharmacological — is predominantly symptom-directed: it may reduce pain and improve short-term function, but does not alter the underlying degenerative process. Even arthroscopic intervention has been shown to offer no significant long-term benefit over conservative management in elbow dysplasia, and prior arthroscopy is associated with more severe OA changes and poorer regenerative outcomes [1].
Regenerative medicine offers a disease-modifying rather than purely palliative approach. The goal is not merely to control symptoms but to slow disease progression, support tissue repair, and improve comfort and quality of life over the long term. These conditions will still require ongoing management — but with regenerative therapy, that management addresses the biological drivers of degeneration rather than masking them.
Mesenchymal stromal cells (MSCs) exert their therapeutic effects primarily through paracrine signalling — releasing anti-inflammatory cytokines, trophic factors, and immunomodulatory mediators that create a reparative environment within the joint. Autologous cells eliminate the risk of immune rejection, and culture expansion to defined passage allows delivery of a standardised, viability-tested cell product at a therapeutically meaningful dose.
Same-day stromal vascular fraction (SVF) preparations are clinically inferior: fresh SVF yields fewer than 10% MSCs by cell count, with viable MSC yield falling below 2% following cryopreservation [2]. Culture-expanded MSCs maintain viability exceeding 94% under the same protocol [2]. The wide inter-individual variation in SVF yield makes consistent dosing unreliable [2]. Culture expansion from 5–10 grams of adipose tissue reliably delivers a standardised dose.
Cryopreservation of culture-expanded cells permits retreatment without further adipose harvest, providing a practical long-term strategy suited to the chronic nature of these conditions.
A retrospective study of 245 dogs with chronic musculoskeletal conditions unresponsive to conventional treatment demonstrated statistically significant improvements across all objective outcome measures at every follow-up time window from 0–6 weeks to 49–78 weeks post-treatment. Measures included stance analysis, shoulder and hip range of motion, lumbosacral pressure algometry, and validated owner- and clinician-reported quality of life scores [3].
In patients receiving a single initial treatment, the mean interval before retreatment was required was 451 days [3]. Clinical response commonly extends to 18–24 months before a top-up is indicated. Follow-up MSK ultrasound confirmed regenerative effects in tendinopathy cases, with resolution of pathological mineralisation and restoration of normal fibre architecture [3].
Autologous PRP delivers concentrated growth factors that modulate the joint inflammatory environment and support tissue healing. It can be co-administered with MSCs or used as monotherapy.
